Community Pharmacy Incident Reporting (CPhIR)
CPhIR is an anonymous reporting program designed to empower community pharmacies for Continuous Quality Improvement (CQI).
The CPhIR program meets the reporting program requirements for Mandatory CQI programs in Nova Scotia, Saskatchewan (COMPASS), New Brunswick, Manitoba (Safety IQ), Prince Edward Island, and Newfoundland and Labrador (MedStep NL).
CPhIR contributes to the National Incident Data Repository for Community Pharmacies (NIDR) of the Canadian Medication Incident Reporting and Prevention System (CMIRPS). Incident data from CPhIR and the NIDR is used by ISMP Canada only for the purposes of analysis, shared learning, and recommendations for incident prevention.
